The following application documents are mandatory for all submissions:
- Current Résumé(s)/CVs.
Demonstrate artist’s experience in the following: creating large-scale exterior work, and history of artistic merit through a strong portfolio of past work. (Required – 2 pages max) - Artist Statement. Describe your (or your Artist Team’s) work and point of view as an artist and muralist. (Required - max 500 words).
- Work Impact Statement. Describe your (or Artist Team’s) interest in creating a mural in Washington, DC and how your practice contributes to the local mural landscape. (Required – 300 words max).
- Work Sample Statement. Describe how your provided work samples and overall application materials demonstrate your capacity to complete a mural project. (Required – 300 words max).
- Work Samples. Include up to five (5) images of previous relevant mural artwork for individual artists or ten (10) Images for Artist Teams. All images must be submitted as high-quality JPG files. Image files are uploaded to the Media Viewer on the application portal (Required). See Addendum A for explanation of Work Samples and Support Materials
- Do not use gifs, tiffs, or other image formats, including images embedded into PowerPoint or PDF documents
- If an Artist Team, samples of work must clearly identify each artist’s work samples.
- NEW! Image Identification List Table. Complete the table under each work sample upload with corresponding details to the submitted images within the application. If an Artist Team, clearly define each artist/s’ work samples and the lead artist. Provide all requested details for each image.
Identification List Details:- Artist Name
- Lead Artist/Artist’s Role on the Project (noting section(s) painted)
- Title, Date
- Medium
- Size - Dimensions
- Location
- Project Budget
- Community Engagement
- Professional References.
- Include three (3) professional references that lists names, addresses, phone numbers, and email addresses.
- Compliance Documents. Forms and instructions are available at Resources for Grant Applicants on CAH’s website (Required).
- Certificate of Clean Hands from the DC Office of Tax and Revenue (OTR) dated within thirty (30) days of application. Non-DC Residents are required to obtain a Clean Hands Certificate. Create an account at My Tax DC to fulfill the request. Last four (4) digits must match the four digits on Tax ID document provided below.
- IRS Form W-9 (version March 2024) for the applicant dated and signed within thirty (30) days of application.
- Statement of Certification signed and dated at the time of application.
- Quote from Insurance Carrier(s).
- Grantee shall procure and maintain insurances including, but not limited to, the following: commercial general liability insurance with minimum limits of one million dollars ($1,000,000.00) per occurrence, two million dollars ($2,000,000.00) general aggregate.

Completed applications must be received and uploaded to the grant portal by the Submission Deadline. Failure to submit mandatory documents in the application will result in disqualification.
